HILL AIR FORCE BASE, Utah -- The Davis County Health Department in partnership with the Hill Air Force Base Occupational Medicine Clinic is holding a number of immunization clinics on base through November of this year.
During each clinic, flu, tetanus, pneumococcal, shingles, and COVID-19* (primarily Pfizer, limited supply of Moderna and Johnson & Johnson) vaccines will be offered. At this time, the 75th Medical group is unable to provide COVID vaccines for federal employees.

*To meet the timeline associated with the federal vaccine mandate for federal employees, individuals must have the first Pfizer dose by Oct. 18, first Moderna dose by Oct. 11 or Johnson & Johnson dose by Nov. 8.
Register ahead of time for a clinics at https://webportal.daviscountyutah.gov/App/FluSchedulePublic/home and use the "select clinic" code HILL to find a date and location.
While visting the website, complete and print a consent form and bring it and your insurance card to the vaccine clinic. Forms will be available onsite as well.
